What Chicago Pacific Founders buys
Chicago Pacific Founders is a healthcare investor based in Chicago, with a second office in San Francisco. It reports more than $5.1 billion of assets under management and 27 current portfolio companies. It works in four areas: value-based care for seniors, such as primary care groups and Medicare Advantage plans; buy-and-build platforms in medical specialties like eye care, dermatology, cardiology, dental and veterinary care; AI and tech-enabled services for healthcare; and senior living real estate, which it owns and runs through CPF Living.
The firm is led by former healthcare operators and calls itself founder-focused. In specialty care it usually starts a platform with a founding practice and then adds more practices, as Ascend Vision Partners has done with eye practices across Florida and into Texas. It also makes growth investments in younger healthcare technology companies, such as Attune in June 2026. CPF does not publish revenue, EBITDA or check size ranges, so a smaller practice is most likely to meet it by joining one of its existing groups.

Greymont’s take
How Chicago Pacific Founders fits a seller
CPF suits a physician, dentist or veterinarian who owns a well-run specialty practice and wants help building a regional group, and founders of healthcare services or health technology businesses tied to Medicare Advantage and value-based care. Its specialty platforms work as management services organizations: the practice joins a network that takes on administrative and business support, while clinicians keep control of patient care. A practice owner will most often meet CPF by joining one of its existing groups, such as Ascend Vision Partners in eye care, Marquee Dental Partners in the Southeast or Pieper Veterinary in the Northeast.
CPF's own newsroom has not been updated since 2022, and its recent announcements are mostly technology investments. An owner should ask which of its practice platforms are still adding partners, and how much equity sellers usually roll into the platform at closing.
Questions owners ask
What kinds of healthcare companies does Chicago Pacific Founders invest in?
It names four areas: value-based care for seniors, such as primary care groups and Medicare Advantage plans; specialty practice platforms in fields like eye care, dermatology, cardiology, dental and veterinary care; AI and tech-enabled healthcare services; and senior living real estate through CPF Living.
Does CPF partner with founder-owned practices?
Yes. It calls its approach founder-focused, and its specialty platforms grow by adding independent practices. Ascend Vision Partners, for example, added eye practices across Florida and entered Texas by joining Thurmond Eye Associates, with CPF as its equity partner.
Does CPF own senior living communities?
Yes. CPF Living, launched in 2014 with operating partner John Rijos, owns and runs independent living, assisted living and memory care communities. The firm says it operates 47 communities in 16 states, managed by its own affiliate, Grace Management.
